Whether you are a new or existing ACUVUE lens wearer, you can SAVE when you get your
eye exam and purchase ACUVUE Brand Contact Lenses.*

You must complete all steps to receive your rebate check:
- Get an eye exam from an Eye Care Professional
AND purchase the required number of boxes of ACUVUE® Brand Contact
Lenses. Product must be purchased within 90 days of the eye exam.
- Go to
www.acuvue.com/rebates and fully complete the online rebate form. Print the completed
form and attach it to the required number of eligible box tops or call
1-888-565-8474 to have a form sent to you. Please allow 2-3 weeks for delivery of rebate form.
- Attach receipt(s) for the eye exam AND product
purchase. Purchase must be made withing 90 days of eye exam.
- Mail by 9/30/2008 to:

Terms and conditions:
Rebate request must be received at the specified address and by the specified date on rebate certificate.
Purchases must be made 3/1/08 through 9/30/08 and received at the mailing address on or before
9/30/08. Product purchases must be made within 90 days of the eye exam. Limit one rebate per customer,
per offer, per ACUVUE Brand purchase, per yearly eye exam visit. This offer is not valid with any other rebate.
Photocopy of certificate is not valid for redemption. Allow 4-6 weeks for delivery.
No PO boxes, only street or rural addresses are acceptable.
NOTICE TO CONSUMERS: If you are personally
filing the claim for reimbursement from a third-party payor (e.g.
insurance company, employer group, etc.) for the purchase of this
product, your purchase price should be reduced by the value of rebate
amount. If your doctor is filing the claim, you must notify the
doctor’s office of the need to deduct the rebate amount from
purchased price used in calculating the claim.
